New Patients

Paperwork: During your first visit our staff will be providing the necessary forms to be filled in person. To expedite the process, some of the forms are available below to print and complete prior to your visit.

Running on time: We know your schedule is busy and that your time is valuable. If you arrive more that 15 minutes late for your appointment, you will need to reschedule your appointment.

Cancellations: Please call within 24 hours if you are unable to keep your scheduled appointment, or a $50 fee will be applied to your account.

What to Expect on Your First Visit

Patient Forms

Please print and complete the patient form(s) below. If you have a work-related injury, please also complete the workers’ compensation form below.

New Patients: Forms need to be filled out in person during your first visit.

Bring these forms with you to your appointment. 

Office Policies

We are a teaching practice: Please note that at times you may be seen by a Medical Student during your appointment. The doctors will always come and speak with you about your visit as well. If you are not comfortable with seeing a student, please notify the nurse at the time of the visit and we will honor your request.

  • Appointment Cancelation Policy: You must call to cancel your appt no later than the business day prior to your appointment and within 24 hours of the appointment time. Canceling your appt the same day or within the 24 hour window, could result in a fee for late cancelation.

    • Missed Appt: A missed appointment fee will be charged as follows:

    • A missed 15-minute appointment, a fee of $50 will be charged.

    • A missed 30-minute appointment, a fee of $95 will be charged.

    • Running on time: We know your schedule is busy and that your time is valuable. Unfortunately, if you arrive more than 15 minutes late for your appointment, you will need to reschedule your appointment.

  • Refills: It is best to call your pharmacy and request they send an electronic med refill request to your provider. This will expedite the, process of your refill being completed.

  • Referrals: In network referrals will be processed if patient has been seen by a Wrightstown Family Medicine Provider within the previous 3 months, regarding the diagnosis for which the referral is being made. If it has been more than three months, you will need an appt with one of our providers prior to the referral being processed.

  • Calling with medical questions: When you call, please leave a message with the nursing dept. They will inform the provider of the concern or question and will return your call or address the issue once the provider gets back to them. All calls should be answered no longer than the end of the following business day.

  • Outstanding Balance: All outstanding must be paid when making an appt. If you arrive for an appt and there is an outstanding balance it will need to be paid prior to seeing the Provider.

Insurances Accepted

Wrightstown Family Medicine accepts most commercial insurances, Medicare advantage plans, and liability insurances for Motor Vehicle Accident and Worker’s Compensation cases.  We do not accept Medicaid or any Medicaid-products.  Please check with your insurance carrier to make sure we are in your network. Insurance companies have many different plans and packages under their company name umbrella which makes it virtually impossible to list them all.

       Example:  Three different patients could all have “Aetna” insurance, but very different Aetna packages which creates     

       variations of coverage.

Please refer to our Wrightstown Family Medicine group National Provider Identification Number (Group NPI #  1740473628 ) when calling your insurance provider to confirm your coverage and benefits.  Please note, it is ultimately the patient’s responsibility to confirm their specific insurance package is participating or non-participating when you are seeking services from a provider office. â€‹â€‹â€‹â€‹â€‹

Referrals

Please leave a message with the Front Desk with details of the referral you may need for a specialist. Referrals usually take 24 to 48 hours to go through the system. You must leave the following information in your message:

  • Name of office and doctor where your appointment is scheduled

  • Phone number of the specialist

  • When your appointment is scheduled

  • NPI number. We cannot put a referral into the insurance system if we do not have the NPI number.
         This information should be given to you by the specialist

  • Diagnosis code – given by the specialist as well

  • The best contact number for you in case of any issues that may arise

Prescription Refills

Prescription refills will be done during business hours only. All messages left for refills will be done the next business day if applicable. All written prescriptions must be called in for a refill 3-5 business days prior to pick up. No prescriptions will be written the day that are called in. Please make sure you have enough medication on hand at all times.
